Please select an appointment time that suits you in the booking form below. You will receive a confirmation email following your booking, and our electronic information & consent form will be sent to you via email prior to your appointment.

Please note that a cancellation fee of 100% of the fee will be charged if you fail to attend your appointment at short notice (i.e. with less than 24hrs business day's notice). Unfortunately rebates can not be claimed for missed appointments.
